FSW has an opening for a full-time Recovery Specialist/Case Manager to work with chronic mentally ill adults. Applicants must have a history of employment in social services working with mentally ill adults, and possess available, reliable transportation. BA required. Bilingual skills and those who have experienced recovery in any sense, e.g., substance abuse, mental health, domestic violence, etc., a plus.

Basic Function:
Under the regular supervision of the Program Director and Clinical Supervisor, the Recovery Specialist/Case Manager helps plan for and coordinate the individual's service plan designed to increase functioning and community integration.

Responsibilities include:

  1. Monitor, review and evaluate the adequacy of services/interventions in meeting the clients' basic community living needs.
  2. Provide rehabilitation services to clients within a range of modalities including individual, group and family intervention.
  3. Develop a service plan with client and other team members which includes specific goals and objectives.
  4. Help clients establish and/or maintain linkage with significant community resources (social, vocational, medical, residential and educational).
  5. Involve clients in activities to increase his/her interest and motivation to actively participate in choosing and/or preparing for valued community role.
  6. Assist clients to make informed choices about the community living, learning, social and/or working environments she/he has identified.
  7. Involve clients in determining which critical skills are needed and wanted for functioning in her/his community goal environment and require an intervention
